Domestic Partner Coverage

Eligibility Criteria

A domestic partnership is defined as one in which both partners are:

18 years of age or older, unmarried and not related by marriage or blood in a way that would bar marriage: residing together; involved in a committed (lifetime) rather than casual relationship and mutually interdependent financially. The partners must be each other’s sole domestic partner for a period of at least six months.
The application process requires that submission of documentation verifies domestic partner registration with either New York City (resident) or New York State (non city residents).

Please complete the Change of Benefit Status form and submit along with a copy of your New York City Certificate of Domestic Partnership (for New York City residents) or copies of the New York State Affidavits of Domestic Partnership to the Association.
