Annual Health Physicals with Bloodwork and Ultrasounds Provided by Acclaimed Mobile Health is coming for the Hard Working Members of the NYSCCA!

ALL COMMAND MEMBERS are welcome to participate in this year's Health and Wellness Program by getting their Annual Physical done at your office! Acclaimed Mobile Health specializes in making it easier to go to the doctor for a health & wellness check, so make sure to take advantage of this opportunity, it may save someone's life!
Acclaimed Mobile Health has been bringing their services to many municipalities to include 35 School Districts, Police Departments, Fire Companies, Bus Depots, Courts, Town and City Halls, and DPW Yards in NYC, Suffolk & Nassau Counties for over 10 years, and continues to have success providing their services at the workplace. Their comprehensive health check has had great impacts on all participating members and their families as well as your counterparts in other Municipalities, Fire and Police Departments, and School Districts across Long Island and beyond!
If you haven't been to the doctor for a well visit or annual physical for any extended period of time, their services provide Multiple Specialties and preventative testing that would take 3 or 4 appointments to complete on your own. Best of all, they bring it to us in the convenience of our workplace!

Some Reminders For The Command:

All patients will receive a confirmation link via text and email before their scheduled appointment
All patients that have made their appointments will receive a reminder the day prior to their appointment
Please setup & login to your portal, complete all of your paperwork and authorizations prior to your appointment it will save about 10min!
Please bring your ID and Insurance Card to your appointment
Please arrive promptly for your appointment, in order to maintain the continuity of the schedule, thereby ensuring no wait times for our patients
All results will be uploaded into the patient portal within 3-7 business days
All patients with an abnormal result or any irregularities will be notified
Bloodwork for 2024:
Acclaimed Mobile is now offering three options for Bloodwork:
PLEASE FAST 6HRS PRIOR TO APPOINTMENT FOR ACCURATE BLOOD WORK RESULTS
- The Patient has blood drawn at the time of their physical / Patient is responsible for their NORMAL LAB COPAY between themselves and the lab (Quest or LabCorp)
-The Patient requests a prescription to have their blood drawn on their own / Results will be uploaded to their Acclaimed Mobile Patient Portal (Normal Lab Copay Applies)
- The Patient elects to not have blood drawn
Billing/Out of Pocket:
If any staff member receives a bill other than a copay please send a copy to the bill will be processed and removed from the patients account
One single copay of $25 is the responsibility of the patient for all diagnostic testing / whether the patient receives 1 ultrasound or 6, there is only one copay for all in the amount of $25
If the patient elects to do bloodwork, the patient will be responsible for their normal copay between them and the lab.
